GENERAL PURPOSE: Provides individualized assistance, support and comprehensive coordination of care for patients and their families/caregivers, to eliminate barriers to timely care, facilitate flow through system, facilitate interactions with care team, increase patient and provider satisfaction, maintain point of contact with providers and serve as first point of contact for patients and families. FT (40 hours) position available: Monday – Friday, day shift. URMC GI Practice is in search of a driven, team-oriented nurse to join our multidisciplinary team providing focused, sub-specialty care to our IBD (Inflammatory Bowel Disease) patient population. This role helps patients navigate the complex medical needs in the management of Ulcerative Colitis and Crohn’s Disease. The IBD Nurse Navigator will develop and implement a care management plan based on patient/provider developed goals and objectives to promote improved health care outcomes and quality of life. This role involves managing a list of high-risk IBD patients, monitoring medication trough levels, managing infusion schedules, providing education and support, and telephone triage/ in-basket management. The IBD Nurse Navigator will work effectively as a member of the health care team by collaborating on population health management, complex care and transitional care management. Requirements

• Bachelor’s degree
• 2-3 years GI nursing experience
• Strong organizational and communication skills
• Experience working within the electronic medical record
