About The Position

The purpose of the Clinical Social Worker 3 (MSW) position is to provide both direct patient care to patients and families and assist in the coordination of team delivered services. The Clinical Social Worker 3 will complete a psychosocial assessment; develop treatment plans that are congruent with the overall health care plan; provide psychosocial intervention; educate patients, their family members, staff and social work students; facilitate compliance with treatment; work with the community to provide needed services; and coordinate discharge planning activities. The new Stravitz Sanyal Liver Institute at VCU is looking for a new licensed clinical social work to join their team! This Institutes goal is to provide a holistic approach to navigating complex chronic conditions such as Cirrhosis, Alcoholic Hepatitis, and Fatty Liver Disease. This specific social work role will support the multidisciplinary team in treating patients living with Alcoholic Liver Disease. This team will rely on this clinician to help mitigate barriers to treatment including assessment and coordinating treatment needs for mental health and substance use issues, providing crisis intervention in clinic, engaging patients in advance care planning conversations, etc. This clinical social worker with meet directly with patients and their caregivers face to face in clinic with their team. Additionally this team member will job a team of social workers who support various clinics throughout the Central Virginia area.

Requirements

  • Licensed Clinical Social Worker in the State of Virginia preferred
  • CSW Level 3: minimum of three (3) years of clinical social work experience with at least one (1) year in a specialty practice area
  • Master of Social Work Degree from a Council on Social Work Education accredited program

Nice To Haves

  • Clinical social work experience in an acute care setting preferred
