Performs comprehensive psychosocial assessments, including evaluating patient's understanding, motivation, mental status, employment, disability, insurance coverage, compliance, social history, substance use, functional status, caregiver, and transportation plan. Collaborates with the multi-disciplinary team to establish and document patient goals and treatment plans. Serves as a consultant and educator to the multi-disciplinary team regarding psychosocial components of illness, disability, and hospitalization. Participates in initial patient orientation and provides ongoing education on transplant, living donation, and VAD processes. Utilizes advanced competence to perform crisis intervention, including follow-up to assess adjustment, caregiver concerns, mental status, and making referrals as necessary. Assists in patient advocacy, resource brokering, and information and referral services. Provides support and education surrounding end-of-life issues and advance directives. Facilitates ongoing support group meetings for transplant patients and their families. Attends and participates in transplant team meetings, including patient review committee, hospital interdisciplinary rounds, discharge planning, and Quality Assurance Process Improvement (QAPI). Proactively initiates quality improvement ideas to improve patient care experience, health of populations served, and reduce clinic care costs. Assists with onboarding, training, and professional development of LCSWs and coordinates learning opportunities for team members. Maintains current knowledge of Medicare, Medicaid rules, and legislation affecting transplant patients.

  • Clinical Social Worker License (LCSW) - EV Accredited Issuing Body
  • Clinical Transplant Social Worker (CTSW) - EV Accredited Issuing Body
