About The Position

CO MAT/Ambulatory Care Pharmacist (Medication for Opioid Use Disorder Pharmacist) Albertsons Companies / Safeway and SAV-ON Pharmacy Locations – Pueblo County, CO Pay Rate: $69.03 Full-Time Clinical Position – 30 Hours/Week Unique Ambulatory Care/Advanced Clinical Practice Role Are you a pharmacist who excels in progressive, patient-centered clinical practice? Do you want to deliver care at the top of your license, build trusted relationships with vulnerable communities, and lead one of the first pharmacist-driven MOUD (Medication for Opioid Use Disorder) programs in the state? If you’re passionate about advancing access to care and transforming the community pharmacy into a clinical care destination, the CO MAT Pharmacist position at Albertsons Companies offers a rare opportunity to practice in a dedicated ambulatory-care–style role , focused entirely on clinical service delivery—not retail dispensing. This is a grant-funded pilot position , and ongoing employment beyond the grant period of one year may depend on continued funding and successful achievement of performance goals.

Requirements

  • Have earned a Doctor of Pharmacy degree or completed at least 5 years of experience as a licensed pharmacist
  • Active Colorado pharmacist license in good standing.
  • Carry adequate professional liability insurance as determined by Colorado Board of Pharmacy
  • Strong clinical decision-making skills and comfort delivering direct patient care and practicing under statewide protocols or collaborative practice agreements.
  • Possess ability to concentrate and deal with frequent interruptions

Nice To Haves

  • Prior experience in ambulatory care, behavioral health, substance use treatment, or chronic disease management
  • Immunization certification
  • Bilingual proficiency (Spanish) to support community engagement in Pueblo County
  • Experience working with underserved or at-risk populations

Responsibilities

  • Perform comprehensive clinical assessments, including withdrawal evaluations, treatment screenings, on site point-of-care urinalysis testing and initiation/adjustment of MOUD therapy under Colorado protocol.
  • Provide ongoing patient follow‑up focused on adherence, symptom management, therapeutic response, and routine monitoring (e.g., vitals, labs).
  • Deliver harm‑reduction services, including naloxone furnishing/prescribing and overdose‑prevention counseling.
  • Coordinate patient care across behavioral health, medical providers, prescribers, and community organizations while assisting with social‑determinant‑related needs.
  • Maintain accurate documentation for required program reporting and contribute to quality‑improvement, outreach, and patient‑engagement efforts.
