Overview Under the direction of the Director of Assisted Living and Care Management, the Licensed Practical Nurse is responsible for providing for the health care needs of Duncaster residents in their apartments. This position functions as a professional caregiver who is accountable for implementing and evaluating all aspects of resident care, including physical, psychosocial, and environmental needs. The LPN acts as the charge nurse and works to resolve issues and concerns of CNAs.

Requirements

  • Valid LPN license in the state of Connecticut.
  • A minimum of 1 year of full-time clinical experience in nursing is required.
  • Must be able to speak, read, and write the English language in an understandable manner.
  • Comprehensive knowledge of principles of nursing, physical evaluation skills, geriatric nursing care, preferably in outpatient services.
  • Strong organizational and administrative skills.
  • Strong customer service skills.

Nice To Haves

  • Experience with MatrixCare or other electronic medical records is preferred.

Responsibilities

  • Carries out the care plan for the residents and instructs the assisted living aide of services to be provided.
  • Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the neighborhood.
  • Transfers and discharges residents as required.
  • Completes data collection as necessary based on the resident's condition.
  • Provides prompt response to changes in resident conditions to include communication with physician or practitioner and family or responsible party as indicated.
  • Complete accident/incident reports as necessary.
  • Refers to appropriate professionals or agencies whenever the condition necessitates ensuring that if the resident's condition is no longer chronic and stable, services of a licensed home care agency are engaged, or other appropriate arrangements are made.
  • Manages treatments as prescribed and medications as prescribed to include verifying orders, pre-pouring or working with pharmacy managing pre-pour, administering and documenting; communicates with MD or practitioner when indicated.
  • Responds to calls for aid to include evaluation for injury, completion of documentation and communication with appropriate parties, and adjustment to plan of care if indicated.
  • Provides accurate and timely regular documentation on 24-hour reports and resident chart of interventions, changes in condition, referrals to other services, notifications of MD and/or family, etc.
  • Utilizes standard precautions at all times and follows transmission-based precautions when appropriate. Reports any potential exposure or hazardous conditions to the charge nurse promptly. Demonstrates skills specific to providing care to residents affected by dementia.
  • Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary.
  • Assigns tasks and delegates duties, when required, to CNAs.
  • Provides mentoring and conflict resolution including de-escalating conflicts and mediation of issues that arise between staff members, residents and families.
  • Complete annual education requirements.
  • Performs other duties as assigned.
