Requirements

  • Master's Degree in Social Work (MSW) required
  • 2 years of experience in psychosocial and therapeutic assessments and interventions required
  • BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
  • LCSW - State of California Licensed Clinical Social Worker required

Responsibilities

  • Serves as a preceptor and provides individual and group mentorship for unlicensed Social Workers and social worker interns in accordance with regulations of relevant State of California licensing boards.
  • Collaborate with multi-disciplinary team in the coordination of care by evaluating, assessing and monitoring the patient care plan.
  • May provide functional direction to unlicensed social work staff and social work interns.
  • Participates in the coordination of transfers to alternative level of care as appropriate, including acute psychiatric facilities.
  • Provide comprehensive assessments and referrals, crisis intervention and care management.
  • Collaborate with multi-disciplinary team and other internal and external personnel to promote an awareness of environmental and psychosocial needs and to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community.
  • Have excellent communication and problem-solving skills.
  • Participates in interdisciplinary planning, case conferences, grant requirements, researches and/or quality improvement projects as needed.
