About The Position

This position is responsible for maintaining a specialized caseload of vulnerable consumers who have been determined (by a Protective Service Intake) to be in an abusive, neglectful, abandoned or exploited situation. As an Aging Care Manager 3 (ACM3), you are required to make critical decisions and encourage consumers who are in need of making a change in their lives. The ACM3 is responsible for timely documentation and must possess initiative and problem-solving skills, pay close attention to details, conduct assessments and investigate within the required time frame. You can expect to coordinate medical, social, psychological or functional evaluations for assigned consumers. This includes appearing before the County Judge for intervention hearings to testify or for a behavioral health commitment and arranging, if needed, a representative payee to protect the consumer's financial assets/resources. Your extensive knowledge of the community resources available to assist consumers is a must. Working independently and the ability to work and respond well in an emergency setting is essential. Additionally, you may be required to carry a caseload of consumers who are not classified as needing Protective Services or who are not classified under the law as protective service consumers. Requirements

  • One year as an Aging Care Manager 2 or Aging Program Assessor; or
  • One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience.
  • You must meet the PA residency requirement.
  • You must be able to perform essential job functions.
  • You must pass a background investigation.
  • A conditional offer of employment will require a drug screening.
  • This position falls under the provisions of the Child Protective Services Law.
  • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
