About The Position

The Support Broker will support members enrolled in Agency Brokerage Department to lead a fulfilling Self-Directed life. Agency Support Brokers work with Self-Directed Participants/Designees to create a Budget and Staff Action Plans that are person centered. Assisting the participant in developing and maintaining a Circle of Support/Planning Team and assisting in planning and directing team meetings (this includes: assisting the participant in scheduling and facilitating team meetings; identifying new team members; identifying and assigning roles and tasks for Circle of Support/Planning Team members; assisting with the overall functioning of the Circle of Support/Planning Team; ensuring team members are freely chosen by the participant; and the team is operating in the participant’s best interests and within the principles of Self-Determination). Ensuring that at least two face-to-face Circle of Support/Planning Team meetings occur each year. Ensuring completion of and updates to Staff Action Plans for self-hired Community Habilitation and Supported Employment (SEMP) staff for participants who receive such services. Assist in developing a comprehensive Self-Directed Budget that is consistent with the Life Plan (LP) and making community connections if needed. Assist with amending any changes to the Self-Direction Budget and plan, if needed. Ensure the Self-Directed Budget and Plan is Person Centered. Provide education and guidance to the entire Circle of Support Team on the regulations set forth by Office for People with Developmental Disabilities and New York State. Work with the Circle of Support to identify connections within the community. Assisting the participant with properly documenting services according to applicable federal and state regulations and policy (including assisting the participant with reviewing and submitting documentation to the FI, such as: employee time sheets; monthly summary note; mileage and expense reimbursement forms; and all o Monitor the reimbursement to ensure that the Self-Direction Participants budget does not exceed the Personal Resource Account (PRA). Attending the participant’s Life Plan reviews and assisting the participant with reviewing and updating his/her Life Plan as requested. Assisting the participant with hiring and retaining appropriate support staff (including assisting the participant in recruiting, interviewing, hiring, scheduling, and supervising self-hired staff, and assisting the participant with identifying and retaining adequate backup staffing to ensure availability as needed by the participant). Ensure all members records are following Office for People with Developmental Disabilities and New York State regulations and policies, conducting quarterly reviews. Ensure effective communication with immediate Supervisor and Circle of Support Team. Immediately report alleged, observed, or suspected abuse and/or neglect. Ensure effective conflict resolution is maintained when issues arise in the Department. All other assignments deemed necessary by the Team Leader,

Requirements

  • Age Requirement: Minimum age of eighteen.
  • Experience: 1 year of experience working with people with Intellectual Disabilities and OPWDD preferred
  • Educational Qualification: Highschool or GED, required
  • Certifications Support Broker Authorization number preferred. However, must be obtained within 6 months of hire
  • Person-centered thinking: Demonstrates the basic structure of services available for people with intellectual disabilities, works to meet a person’s needs and desires, and can advocate for additional services if needed.
  • Professionalism: Demonstrates strong commitment to meeting the needs of participants, designees, families, and advocates. Considers the impact on participant service satisfaction when acting and carrying out one’s own job responsibilities. Takes personal responsibility for resolving service problems brought to one’s attention.
  • Effective Communication: Engage in effective verbal and written communication with the person being supported, designees, and advocates through appropriate tone of voice, first-person language, and modeling. Demonstrates caring through body language, tone, and providing adequate time for communication.
  • Effective Teaching: Leads and engages to maximize a person’s learning through alignment of the Life Plan and Staff Action Plan. Continuously evaluate the service plans and make recommendations as needed. Demonstrates a shared responsibility with team members by engaging and participating in the person’s Circle of Support meetings.
