About The Position

Position Overview: The Aftercare coordinator is a crucial member of our addiction treatment facility's team, responsible for supporting patients as they transition to the next phase of their recovery journey. The primary objective of this role is to work with Case Management develop transition and aftercare plans. The Aftercare coordinator will facilitate and coordinate ongoing care and support services for patients, ensuring their continued success in maintaining sobriety. In addition to patient-focused responsibilities, this role includes group therapy, case consults and coordination of care with collaterals and patient supports.    This is a full-time position.

Requirements

  • Education: Bachelor's degree in social work, psychology, counseling, or a related field preferred.
  • Experience: Knowledge of addiction and recovery principles, relapse prevention strategies, and community resources.
  • Licensure/Certification: None.
  • Knowledge and Training required at time of hire: Excellent communication, interpersonal, and organizational skills.

Responsibilities

  • Continuing Care Planning: Collaborate with the treatment team, patients, and referents to create a personalized continuing care plan that addresses the unique challenges and goals of each individual.
  • Resource Referral: Identify and connect patients with external resources and support services while adhering to confidentiality and HIPPA guidelines.
  • Monitoring and Follow-Up: Regularly check in with patients to track their progress, address any challenges, and provide ongoing support.
  • Clinical Service Delivery: deliver group therapy to patients as scheduled. Assist with case coverage as necessary and assigned.
  • Crisis Intervention: Be available to provide crisis intervention services, offering immediate assistance to patients when they are at risk of leaving against advice or facing other issues that may interfere with their continuing care.
  • Education and Advocacy: Educate patients about the importance of ongoing recovery efforts and advocate on their behalf to access necessary services and resources.
  • Documentation: complete documentation of timely group notes, complete aftercare and service coordination notes.  Will complete any documentation related to case coverage.   
  • Collaboration: Work closely with therapists, counselors, and medical professionals within the facility to ensure a seamless transition throughout levels of care.
  • Other Duties as Assigned.
