About The Position

The Aftercare Coordinator is an integral member of the Business Development and Admissions teams. The individual shall serve as the principal liaison among external providers, SHH clinicians, and Business Development staff upon the discharge of patients from specific program(s). The primary goals of the position are coordinate with the clinical staff to ensure that patients are discharged to appropriate levels of care, and that the business development team is informed about those discharges. The previous duties are illustrative and not exhaustive. The omission of specific statements of duties does not exclude them from the position if the work is similar or a logical assignment to the position.

  • Knowledge of software programs for personal computer use, especially word processing, email, and spreadsheets.
  • Knowledge and experience of the range and type of services available to mental health clients.
  • Ability to prepare, implement presentations.
  • Strong problem solving skills and the ability to carry out assigned projects to their completion.
  • Strong interpersonal skills, including the ability to communicate effectively both verbally and in writing, establishing and maintaining effective working relationships, gaining cooperation and resolving conflicts.
  • Ability to plan and organize workload, manage deadlines of him/herself.
  • Ability to maintain and demonstrate ethical professional standards and demeanor, including the confidentiality of employee and patient data, patient boundaries, and other sensitive information.
  • Demonstrated competence with, and continued commitment to respect for cultural differences and diversity, protecting patient rights.
  • Recognition of the limits of one’s own professional expertise and the willingness to seek supervision and/or support, as needed.
  • Make decisions with patients’ and agency well-being and best interests in mind.
  • Bachelor’s degree in a field related to Behavioral Health or at least three years of full-time Behavioral Health work experience

Responsibilities

  • Understand the admissions, clinical and aftercare processes through continuous training and completing an orientation of programs
  • Meet and build relationships with aftercare partners and referrers
  • Coordinate with SHH clinicians to identify aftercare partners meeting the clinicians’ recommended post-discharge level of care
  • Schedule, facilitate and/or participate in telephone discussions between Silver Hill clinicians and aftercare partners, as appropriate. This would include but not limited to doctors, social workers, outpatient providers, and sober living communities.
  • Assist SHH clinicians in involving patients and families in the determination about optimal continuing care options available.
  • Meet with patients to facilitate required intake calls, as needed.
  • Attend conferences for physicians, psychologists, and other clinical providers when appropriate
  • Keep detailed records of all contacts.
  • Build and maintain positive working relationships with providers outside of Silver Hill Hospital.
  • Maintain knowledge of new developments in the mental health field, anticipating potential impacts on patients and adapting continuing care strategies accordingly.
  • Develop strategies for increasing opportunities to meet and talk to mental health providers
