About The Position

We are seeking an experienced After-Hours Nurse Manager (RN or LPN) to provide clinical triage, referral oversight, and on-call coordination for multiple skilled home health offices during after-hours periods. This role is ideal for a confident, independent nurse who excels in clinical decision-making, phone triage, and operational coordination. This position includes on-call availability pay plus hourly compensation for time worked. Pay: 45k-50k annually (combination salary and on call per diem stipend) Schedule On-call coverage from Friday evening through Monday morning This is an on-call role, not continuous active work Work is intermittent based on call volume and referrals

Requirements

  • Current RN or LPN license in good standing (Florida)
  • Home health experience strongly preferred
  • Strong clinical assessment and phone triage skills
  • Ability to work independently and make sound decisions after hours
  • Excellent communication, organization, and documentation skills
  • EMR experience required (KanTime preferred)

Responsibilities

  • Serve as the primary after-hours clinical leader for patients, caregivers, and on-call staff
  • Triage incoming patient calls via phone and determine appropriate next steps
  • Provide clinical guidance when issues can be safely managed by phone
  • Determine when in-person visits are needed and coordinate care with on-call clinicians
  • Enter all after-hours calls and visit needs into the EMR for on-call tracking
  • Monitor after-hours emails, faxes, and referral sources for new skilled home health referrals
  • Review, process, and document referrals received on evenings and weekends
  • Schedule urgent or start-of-care visits with on-call clinicians when appropriate
  • Review Friday handoff reports to anticipate weekend needs and pending issues
  • Prepare and deliver a Monday after-hours activity report summarizing calls, visits, referrals, and follow-ups
  • Ensure accurate documentation and compliance with agency, payer, and regulatory requirements
  • Escalate complex or high-volume situations to backup clinical leadership as needed
