About The Position

Helms Home Care is seeking a dedicated After Hours Patient Care Coordinator to join our growing team. This role is vital in supporting patients receiving home infusion therapy and ensuring smooth coordination between pharmacy vendors, nursing staff, and patients.

Requirements

  • Must work onsite (no remote work available)
  • Proficiency in Microsoft Office Suite (Excel, Word, PowerPoint) and Adobe Acrobat Pro
  • Strong computer skills, including accurate typing and efficient data entry
  • Excellent written and verbal communication skills; ability to communicate professionally by phone and email
  • High school diploma or equivalent

Nice To Haves

  • Experience with IV medications, infusion therapies, or pharmacy-related work
  • Associates Degree (Preferred)
  • Customer Service: 2 years (Preferred)
  • Medical Terminology: 1 year (Preferred)
  • Computer Skills: 2 years (Preferred)

Responsibilities

  • Accept and process new patient referrals from various vendors
  • Schedule home infusion visits and maintain accurate records in the scheduling system
  • Communicate with pharmacy vendors and nursing staff regarding patient care, supply needs, and scheduling updates
  • Support overall patient care coordination with professionalism and attention to detail
