About The Position

Under the supervision of the Housing Coordinator, provides a variety of paraprofessional mental health services to consumers with regard to their housing or residential needs. Works in close coordination with the Housing Support Team’s AFC Specialist, the AMHS treatment teams, and the residential teams in developing and carrying out person-centered planning as it relates to Federal and State regulations (e.g., HCBS or LARA). Will assist in reviewing consumer housing and residential level of care needs, co-facilitating groups, transportation, Wellness Coaching community inclusion activities, data entry, and completing timely service documentation. Responsible for carrying out all activities of the program/sub-unit in such a manner that fulfills CMHA-CEI’s mission, policies and procedures.

Requirements

  • Possession of a High School diploma or G.E.D. equivalency is required.
  • A minimum of six (6) months experience working with persons with mental illness is required.
  • Ability to communicate accurately and effectively both in writing and verbally is required.
  • Possession of a valid Michigan driver’s license and access to reliable means of transportation for job related use required.
