About The Position

The incumbent is part of a global public health organization and supports the Florida Department of Health in Citrus County (FDOH Citrus) in its mission to protect, promote, and improve the health of all people in Florida through integrated state, county, and community efforts. The incumbent is expected to embody the agency’s core values: Innovation, Collaboration, Accountability, Responsiveness, and Excellence.This position provides direct disease management and public health intervention services across multiple programs, including Family Planning, STI/STD, HIV/PrEP, Immunizations, and Communicable Disease/Epidemiology, following established DOH guidelines, protocols, policies, and procedures. The role requires advanced clinical expertise, independent judgment, and the ability to work with diverse populations.

Requirements

  • Public Health Systems: Deep understanding of state, county, and community health structures and services.
  • Clinical Guidelines & Protocols: Familiarity with CDC STI Treatment Guidelines, ASCCP guidelines, Title X Family Planning Manual, and FDOH protocols.
  • Disease Management: Knowledge of chronic and acute conditions, especially in adolescent and adult populations.
  • Preventive Health: Expertise in immunizations, screenings, and health promotion strategies.
  • Reproductive Health: In-depth knowledge of family planning, contraception, infertility, and preconception care.
  • HIV/STI Care: Understanding of HIV prevention (PrEP), testing, counseling, and treatment protocols.
  • Cultural Competency: Awareness of diverse social, economic, and cultural backgrounds and their impact on health behaviors.

  • Advanced Clinical Practice: Proficient in physical exams, diagnostics, treatment planning, and medication management within APRN scope. Ability to insert and remove LARCs.
  • Patient-Centered Care: Skilled in shared decision-making, counseling, and education tailored to individual needs.
  • Diagnostic Proficiency: Ability to select and interpret lab tests, imaging, and other diagnostics.
  • Documentation: Accurate and timely documentation in electronic health systems (e.g., HMS, Florida SHOTS).

  • Time Management: Efficient clinic flow management with a minimum productivity standard (2 patients/hour).

  • Education & Advocacy: Promote healthy behaviors and advocate for public health initiatives.
  • Compliance & Ethics: Uphold ethical standards and ensure compliance with all relevant health regulations.
  • Continuous Learning: Stay current with clinical guidelines, immunization updates, and public health trends.
  • Clear and active license to practice as an Advance Practice Registered Nurse (APRN) in accordance with Florida Statute 464.

Responsibilities

  • Manage care for adolescent and adult clients with chronic or acute conditions, including well care and screenings.
  • Provide medical and nursing diagnoses and formulate treatment plans.
  • Perform comprehensive physical examinations and evaluate medical histories and findings.
  • Select and interpret diagnostic tests (e.g., urinalysis, blood counts, imaging, cultures).
  • Monitor therapies and medications, adjusting treatment as needed.
  • Coordinate continuity of care and make referrals for services outside APRN scope.
  • Educate clients and families on healthy behaviors (e.g., diet, exercise, substance use, sexual health).
  • Counsel on smoking risks and cessation resources.
  • Maintain clinic flow efficiency with a minimum productivity of 2 patients per hour.
  • Document care and communications in real time using HMS; complete orders before seeing the next client.
  • Deliver Title X Family Planning services within APRN scope using the Florida Title X Clinical Manual.
  • Assess reproductive life plans and provide patient-centered care using shared decision-making.
  • Maintain certification and proficiency in insertion/removal of long-acting reversible contraceptives (LARC), including implants and IUDs.
  • Perform Pap smears and manage results per ASCCP guidelines.
  • Provide infertility and preconception services.
  • Offer pregnancy testing and counseling.
  • Provide non-directive counseling and informed consent for contraceptive methods.
  • Counsel on tubal ligation and vasectomy options.
  • Assess, evaluate, and enroll eligible clients.
  • Order diagnostics (e.g., mammograms, Pap tests) and labs.
  • Communicate with program representatives and follow up on all results.
  • Manage abnormal results, including referrals for biopsies, MRIs, oncology, and colposcopy.
  • Provide routine STI screening and treatment, including HIV, per CDC guidelines.
  • Offer pre- and post-test HIV counseling.
  • Collaborate with Disease Intervention Specialists to ensure appropriate treatment.
  • Offer PrEP to at-risk clients.
  • Offer PEP as indicated.
  • Evaluate and treat clients of all ages presenting with STI symptoms or requesting testing.
  • Review lab results promptly (ideally within 1 business day, no more than 2).
  • Notify clients of positive results (may delegate to RN).
  • Assist with HIV test-and-treat services per protocol.
  • Administer immunizations per CDC and ACIP schedules.
  • Promote recommended (non-required) immunizations.
  • Record immunizations in Florida SHOTS.
  • Stay current on immunization updates and changes.
  • Assess and administer rabies post-exposure prophylaxis per FDOH guidelines.
  • Evaluate and select educational materials for clients.
  • Working in collaboration with the Epi Team prescribe prophylaxis for contacts to communicable diseases including but not limited to meningitis and pertussis.
  • Participate in community events and build relationships with partners.
  • Engage in quarterly peer chart reviews.
  • Complete mandatory trainings on time.
  • Attend in-service education programs.
  • Perform other duties as assigned within APRN scope.
  • Maintain confidentiality and adhere to Citrus County Health Department policies.
  • Attend annual confidentiality and security trainings.
