About The Position

At Chestnut Family Health Center, we serve individuals and families across the lifespan — from pediatrics to geriatrics — many of whom face significant barriers to care. We’re looking for a mission-driven Advanced Practice Provider (APRN or PA) to join our team of dedicated professionals working in an integrated primary care and behavioral health setting. Requirements

  • RN plus Master’s degree in nursing.
  • Licensure as an APRN in the State of Illinois.
  • License in good standing with the ability to bill for evaluating and prescribing medications.
  • Active buprenorphine waiver or willingness to obtain.
  • Fully executed Collaborative Agreement with a Chestnut-approved prescriber will be required.

Nice To Haves

  • Dual Missouri licensure preferred.
  • Previous psychiatric and primary care prescribing experience preferred.

Responsibilities

  • Provide comprehensive primary care services including acute, preventive, and chronic disease management for pediatric through geriatric patients.
  • Conduct health assessments, physical exams, and develop treatment plans in collaboration with a multi-disciplinary team.
  • Serve as a prescriber in accordance with Illinois law and agency policy; participate in collaborative physician meetings and case discussions.
  • Contribute to integrated care by participating in team huddles, staffing meetings, and consultation activities.
  • Use electronic health records (EHR) for documentation and meet expected productivity, quality, and documentation standards.
  • Assist in maintaining compliance with HRSA, Joint Commission, and other regulatory/accrediting bodies.
  • Support continuous quality improvement and program development efforts.
