About The Position

Advanced Practice Registered Nurses (APRN) at Premier Community HealthCare Group (PCHG) provide patients with primary care and maintain continuity of care within our Family Medicine/ Pediatric Departments. We strive to serve our community in establishing, diagnosing, and treating acute and chronic conditions. Our goal is to provide health promotion, prevent disease, and strive for healthy communities. APRN providers will perform general medical procedures, both diagnostic and therapeutic, using minimally invasive techniques.

Requirements

Master's degree from an accredited Nurse Practitioner Program with a clinical specialty area of Family Medicine or Pediatrics.
Current APRN license in Florida.
Current Basic Life Support (BLS) certification.
Obtain and maintain Epic certification/ annual compliance training throughout employment.

Ability to lift 20 lbs. regularly and 30 lbs.-50 lbs. occasionally.
Ability to sit for long periods of time.
Direct exposure to computer screen.
May be exposed to contagious/infectious diseases.
Ability to travel between PCHG sites to provide coverage, if necessary.

Nice To Haves

Current Drug Enforcement Agency (DEA) License strongly preferred or ability to obtain.
Minimum of three (3) years’ experience as a Nurse Practitioner preferred.

Responsibilities

Assure comprehensive healthcare needs are identified and treated for age-specific populations in collaboration with the supervising physician. These duties are to be performed within the Nurse Practitioner’s scope of practice and in accordance with PCHG’s policies.
Provide education and give health management information and illness prevention techniques for the promotion of community health.
Communicate effectively with all members of the Care Team to ensure smooth handoffs and a positive patient experience.
Consult and coordinate with health care providers/clinicians working at any location.
Mentor and train new providers or clinical support team members as needed.
Work in coordination with the Quality Assurance Team and management to develop and implement a quality assurance program.
Documents in a timely fashion accurate accounts of interactions with patients in Electronic Health Records in compliance with PCHG protocol.
Communicates any concerns about patient care issues or interpersonal difficulties. Follows proper chain of command and proper channels of communication.
Participate in a rotation for afterhours services on-call.
Manage daily and weekly schedule to ensure encounter targets are met and documented appropriately in the Electronic Health Record (EHR).
Review and address tasks daily and maximize productivity.
Ensure confidentiality and HIPAA compliance at all times.
Keep other care team members informed when situations occur that disrupts timely patient flow through site.
Contribute to community give back initiatives and be an advocate for Premier’s mission.
Adhere to patient care needs and the core values of Premier Community HealthCare to a provide best-in-class patient experience.
Work towards attaining department goals and drive Quality Improvement / Quality Assurance (QI/QA).
