About The Position

The Advanced Practice Provider will provide comprehensive palliative care to patients across care settings for the Section of Geriatrics and Palliative Medicine, Department of Medicine, at University of Chicago Ingalls Memorial Hospital. Work collaboratively within a multidisciplinary team, and with additional palliative medicine clinicians, using advanced clinical skills to manage a caseload of patients. Assess, diagnose, initiate, and delegate medical treatments, perform independent assessments and manage care. Provide a model of care that is shaped by theory, expanded by science, research and reflective practice and is identified by a holistic and comprehensive approach to the patients.

Requirements

  • Minimum requirements include a college or university degree from an accredited school/college.
  • Minimum requirements include knowledge and skills developed through a combination of education and a minimum of 2 years of relevant work experience in a related job discipline.
  • Current and/or eligible for State of Illinois licensure/registration required.
  • Eligibility to be privileged and credentialed through the Medical Staff Office (MSO).

  • Previous clinical experience.
  • Once licensed and as needed, apply for a mid-level practitioner controlled substance license and DEA number.
  • Willing to be privileged and credentialed through UCM Medical Staff Office prior to start date.
  • Enter into written collaborative agreement with a collaborating physician upon hire.

Responsibilities

  • Independently manages the care of hospitalized patients within area of palliative care and within parameters of medical staff privileges.
  • Provides co-management of palliative care services within an embedded oncology clinic.
  • Conducts palliative care visits within homes and/or nursing facilities.
  • Obtains relevant patient health history.
  • Studies and interprets patient tests and records.
  • Performs physical exam and record findings.
  • Assists with diagnosis, treatment, and management of acute and chronic health conditions.
  • Prescribes/orders appropriate pharmacologic and non-pharmacologic interventions and develops a treatment plan.
  • Develops an educational plan for patient/family.
  • Orders appropriate consultation/referral (outpatient setting).
  • Determines effectiveness of treatment plan with documentation of treatment provided and patient outcomes.
  • Reassesses and modifies plan with patient/family and treatment team within the parameters of medical staff privileges.
  • Participates in the educational experiences of nursing students, fellows, medical students, residents and other healthcare disciplines.
  • Involved in case conferences, serving as a preceptor and mentor, assisting with curriculum development and instruction.
  • Works closely with physicians, fellows, interdisciplinary staff and research personnel to advance the mission of the Palliative Care team.
  • Participates in the development, implementation, and expansion of the palliative care practice.
  • Assists with care coordination for hospice referrals.
  • Evaluates goals and outcomes taking into account the benefit-versus-burden, safety, quality, and cost-effectiveness for the patient, family and the organization.
  • Develops diagnostic strategies and therapeutic interventions needed to achieve the goals and outcomes of the patient’s plan of care.
  • Performs other related work as needed.
