Heartland Alliance Health’s (HAH) mission is to transform healthcare for the most vulnerable – particularly people experiencing homelessness, mental illness or addictions, or struggling with multiple chronic illnesses – improving health for all and the well-being of our community. We are seeking a compassionate and dedicated Advanced Practice Provider (NP/PA) to join our team in providing high-quality, person-centered care to individuals experiencing homelessness. In this dynamic role, you will work collaboratively with a multidisciplinary team to address the complex health needs of our participants, including primary care, chronic disease management, and mental health support. The Advanced Practice Provider (APP) provides quality primary healthcare services to special populations, emphasizing the promotion of health and the prevention of disease. This role addresses society's demand for accessible and affordable healthcare. The APP works as an integral member of a multidisciplinary team, managing acute and chronic health issues while collaborating with physicians, nurses, and other healthcare professionals. Individual accountabilities and work volumes will be established annually.
