Advanced Practice Provider | Outpatient Primary Care
About The Position
We are seeking an experienced and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our primary care clinic focused on value-based, relationship-driven care. This role is ideal for an APP who believes great primary care is built on time, trust, prevention, and long-term outcomes—not rushed visits and volume-driven metrics. In our clinic, APPs are trusted primary care providers with their own patient panels, supported by interdisciplinary teams and a care model designed to keep patients healthy, engaged, and out of the hospital.
Requirements
- Graduate of an accredited NP or PA program
- Current unrestricted APRN or PA license
- National board certification
- BLS certification
Nice To Haves
- Primary care or outpatient experience
- Chronic disease management experience
- Interest in value-based care and population health
Responsibilities
- Provide comprehensive primary care services including preventive, wellness, and acute care visits
- Diagnose, treat, and manage acute and chronic conditions using evidence-based guidelines
- Develop and oversee longitudinal care plans focused on prevention and chronic disease optimization
- Manage medically complex patients and address social determinants of health
- Prescribe medications and therapies within scope of practice
- Collaborate with nurses, care coordinators, behavioral health providers, and specialists
- Use population health data and quality metrics to close care gaps
- Educate and empower patients to actively participate in their care
