Primary Care- Advanced Practice Provider Saint Francis Family Medicine- Saint Francis- Trinity Health Mid-Atlantic Location: Wilmington, DE --- Position Summary Family Medicine Saint Francis, part of Saint Francis Hospital, is seeking a Board-Certified/Board-Eligible Primary Care Nurse Practitioner/ Physician Assistant to support continued primary care expansion and access initiatives across the Delaware market. Family Medicine Saint Francis offers a range of services, including general primary care, management of acute and chronic illnesses, and preventive health screenings. In addition to general internal medicine, the center provides specialized care, and a multitude of specialties. Requirements

  • NP or PA from an accredited institution
  • Eligible for Delaware licensure
  • Commitment to high-quality, patient-centered care

Responsibilities

  • Provide comprehensive primary care for patients across the lifespan
  • Diagnose acute and chronic conditions
  • Emphasize preventive care and population health management
  • Collaborate with interdisciplinary teams to optimize patient outcomes
  • Maintain timely and accurate documentation
  • Participate in quality and performance improvement initiatives
  • On-call responsibilities (1:3 call schedule), phone-only
