About The Position

Kidney Specialists of Minnesota (KSM) has been providing high-quality care to patients with CKD, diabetic kidney disease, and End Stage Renal Disease (ESRD) since 1978. We are looking for an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our Nephrology team to care for our patients in collaboration with our Board-Certified Nephrologists and multidisciplinary care team.

Requirements

  • Master’s or Doctorate degree in Nursing or Master’s degree in Physician Assistant Studies from an accredited program.
  • Current licensure as a Nurse Practitioner or Physician Assistant in Minnesota.
  • Prior inpatient, intensive care, nephrology, and/or dialysis experience.
  • Current certification in CPR or BLS.
  • Clinical Competence: Applies medical knowledge effectively to provide appropriate, evidence-based patient care.
  • Communication: Communicates clearly and respectfully with patients, families, and colleagues.
  • Professionalism: Demonstrates professionalism, ethical behavior, and respect in all interactions.
  • Teamwork & Collaboration: Collaborates effectively with peers and clinical staff to support team-based care.
  • Practice Management & Efficiency: Manages clinical responsibilities reliably, including timely documentation and patient care tasks.
  • Patient-Centered Care: Demonstrates empathy and consistently prioritizes patient needs in care delivery.
  • Practice Stewardship: Actively contributes to the improvement of the overall practice through formal and informal leadership, quality initiatives, mentorship, or operational support.

Responsibilities

  • Independently collecting patient medical history, determining a plan for patient care, and routinely monitoring follow up and evaluating status
  • Ordering and interpreting diagnostic tests and prescribing medications, as appropriate
  • Providing patient, family and care taker education on disease management and prevention.
  • Coordinating patient care with referring provider/care team as well as primary Nephrologist, as appropriate.
  • Maintaining accurate and timely documentation, adhering to state and federal regulations governing advanced practice, and staying current on evidence-based guidelines, regulatory changes, and payer requirements.
  • Providing patient care through in-person visits and telehealth encounters, ensuring high-quality, patient-centered care across all modalities.
  • Actively engaging with population health management and quality initiatives.
