About The Position

Kidney Specialists of Minnesota (KSM) has been providing high-quality care to patients with CKD, diabetic kidney disease, and End Stage Renal Disease (ESRD) since 1978. We are looking for an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our Nephrology team to care for our patients in collaboration with our Board-Certified Nephrologists and multidisciplinary care team. Advanced Practice Providers perform a vital role at Kidney Specialists of Minnesota. This position is focused on rounding in acute care hospitals, but could also involve rounding at dialysis units and includes: Independently collecting patient medical history, determining a plan for patient care, and routinely monitoring follow up and evaluating status Ordering and interpreting diagnostic tests and prescribing medications, as appropriate Providing patient, family and care taker education on disease management and prevention. Coordinating patient care with referring provider/care team as well as primary Nephrologist, as appropriate. Maintaining accurate and timely documentation, adhering to state and federal regulations governing advanced practice, and staying current on evidence-based guidelines, regulatory changes, and payer requirements. Providing patient care through in-person visits and telehealth encounters, ensuring high-quality, patient-centered care across all modalities. Actively engaging with population health management and quality initiatives. Requirements

  • Master’s or Doctorate degree in Nursing or Master’s degree in Physician Assistant Studies from an accredited program.
  • Current licensure as a Nurse Practitioner or Physician Assistant in Minnesota.
  • Prior inpatient, intensive care, nephrology, and/or dialysis experience.
  • Current certification in CPR or BLS.
