About The Position

Key Responsibilities Clinical Excellence Provide comprehensive primary care for acute and chronic conditions Efficiently manage 23–24 patients per day using 20-minute visit templates Actively address care gaps (HEDIS) and ensure accurate HCC/RAF documentation at every visit Ensure patients receive all appropriate preventive screenings and follow-up care Operational Efficiency Team Leadership: Direct Medical Assistants and Scribes to prep charts, scrub for care gaps, and pend orders prior to visits Chart Hygiene: Maintain a clean desktop with 95% of charts signed and closed within 48 hours Schedule Adherence: Follow visit templates to maintain steady patient flow throughout the workweek Schedule Days: Monday – Friday Hours: 8:00 AM – 5:00 PM (subject to clinic needs) Daily Structure: 7.5 hours clinical time (approx. 23 visits) 1-hour lunch 30 minutes protected administrative time for chart closure

Requirements

Current APRN or PA-C license in the state of practice
Minimum 3 years post-graduate experience in Primary Care, Family Medicine, or Internal Medicine
Comfortable in a high-volume environment (23+ patients/day)

Nice To Haves

Prior experience in value-based care (HEDIS, RAF/HCC coding) strongly preferred
Experience with Athena EMR highly preferred
