The UNC Neurology Neurogenetics program is looking for an Advanced Practice Provider (APP) to join our growing interdisciplinary group. Our team includes neurologists, genetic counselors, geneticists, nurses, and clinic staff. Our core team collaborates closely with multiple clinical Centers of Excellence (CoE) and interdisciplinary clinics. We also collaborate closely across departments, as many neurogenetic disorders impact other systems such as cardiac and endocrine. We share long term care of mitochondrial and metabolic disorders patients of all ages with clinics homed in adult and pediatric clinics, even receiving direct referrals of symptomatic patients (including adults/parents) to us from newborn screening programs. The APP will be involved in all aspects of patient care and education, including the development and institution of personalized, symptom management care plans that leverage medical, surgical and rehab options; facilitating collaborative care for the patient panel across disciplines and departments; hold discussions around advanced directives, hospice/palliative care, and other end-of-life issues as appropriate. The APP will be closely involved in finding and sharing evidence based clinical practices across our group and with collaborating providers. Ability and willingness to engage with lifespan care are key attributes for this position. UNC Neurogenetics clinics see patients of all ages. This APP position will mainly support adult patient populations, but clinic visits with teens are not uncommon and visits with older children are possible. In addition, a neurogenetic diagnosis in one family member may immediately impact multiple other people in both older and younger generations. Comfort with addressing whole family needs and coordinating care across pediatric, adult, and geriatric clinic settings is important. Summary: A Nurse Practitioner functions as part of the interdisciplinary healthcare team in accordance with privileges approved by the credentialing committee to provide high quality, cost effective care to patients within the APPs scope of practice in collaboration with a supervising physician. The APP reflects the mission, vision, and values of the organization, and complies with all relevant policies, procedures, guidelines and other regulatory and accreditation standards.
