About The Position

The Advanced Practice Provider (APP) will provide comprehensive clinical care for the Electrophysiology (EP) and General Cardiology populations located at Fontaine office. This primary location serving patients with cardiology needs. This position serves mainly EP services (75% effort) with the other portion serving General Cardiology (25%). Evaluation and treatment of common EP conditions such as Afib, Aflutter, VT, bradycardia, and patients needing ablations or pacemaker/ ICD placements are core to the practice. In general cardiology services, the APP will treat conditions such as HTN, HLD, CAD and refer patients to other cardiology sub-specialties as needed. The clinic has stress testing, vascular lab, device clinic and echocardiogram on site as well as a medical lab. The APP will deliver evidence-based cardiovascular care tailored to the patient's needs, serve as a patient advocate, and function as a key member of a multidisciplinary team committed to excellence in patient outcomes, education, and compassionate care. Shifts: Monday-Friday, primarily daytime hours; no weekends, holidays or call expected.

Requirements

  • Education: Graduate of an accredited graduate school of nursing or physician assistant program with a master's degree.
  • Experience: Department and unit specific.
  • Licensure: License or license eligible as an Advance Practice Provider (Nurse Practitioner, Physician Associate, Certified Registered Nurse Anesthetist, and Certified Nurse Midwife) as set forth by the state of Virginia. If Nurse Practitioner, license to Practice as a Registered Nurse in the Commonwealth of Virginia is required.
  • Must demonstrate and maintain all credentialing and licensing requirements of UVA Health System, the Virginia Board of Medicine or the Virginia Board of Nursing, and appropriate certifying bodies.
  • Current Basic Life Support (BLS) required.
  • Current PALS/NRP, as required, specific to position.
  • Advanced Cardiac Life Support (ACLS) as required, specific to position.
  • Job requires standing for prolonged periods, frequently bending/stooping, reaching (overhead, extensive, and repetitive); Repetitive motion: computer keyboard.
  • Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 50 - 100lbs.
  • May be exposed to noise, radiation, radioactive materials, blood/body fluids and infectious disease.

Nice To Haves

  • Preferred 3 years experience in Cardiology clinic
  • Prior clinical experience in cardiology, internal medicine, or cardiovascular subspecialties.
  • Experience with outpatient management of cardiovascular conditions.

Responsibilities

  • Administration Managing staff needs and resources while maintaining high standards of patient care and adapting to changes in healthcare policies.
  • Collaborate closely with physicians, hospital administrators, and other healthcare professionals to improve workflow and patient outcomes.
  • Professional Partner with pharmacists to optimize medication therapy and patient adherence.
  • Work with social workers to address behavioral health and social needs.
  • Collaborate with registered nurses and navigators for care coordination and follow-up.
  • Coordinate care with imaging, heart failure, and electrophysiology subspecialists as appropriate.
  • Participate in interdisciplinary case conferences and care planning meetings.
  • Clinical Independently evaluate, diagnose, and manage new and follow-up patients with General Cardiology and complex cardiovascular prevention needs.
  • Perform cardiovascular assessments; order and interpret diagnostic testing; develop and implement individualized treatment plans.
  • Manage cardiovascular risk factors and chronic conditions including hypertension, dyslipidemia, obesity, diabetes, and menopause-related risk.
  • Manage general cardiovascular conditions such as hypertension, coronary artery disease, heart failure, arrhythmias, and valvular disorders.
  • Provide patient and family education regarding disease processes, treatment options, procedural care, and long-term self-management.
  • Coordinate diagnostic testing, referrals, and follow-up care.
  • Document clinical encounters accurately and timely in the electronic medical record (EPIC).
  • Leadership Contribute to fostering a multidisciplinary, collaborative clinic culture.
  • Support quality and program development through adherence to established care pathways and performance metrics.
  • Act as a clinical resource and subject matter expert in women's cardiovascular health.
