About The Position

About this role: As an Advanced Practice Provider (NP or PA), Primary Care (House Calls) you will work with a team of healthcare professionals committed to providing quality care and excellent patient experience. In this role you will provide primary care using your personal vehicles to patient homes throughout Las Vegas. Use of critical thinking skills and strong communication skills are needed. The role of healthcare providers in outpatient settings, particularly in Las Vegas at Intermountain Health, involves a comprehensive approach to patient care. The focus on senior patients highlights the importance of personalized treatment plans tailored to individual needs. Each day, providers conduct six in-home visits alongside one virtual consultation, ensuring accessibility and continuity of care for Medicare patients. This model not only addresses the immediate health concerns of patients but also fosters a supportive environment conducive to long-term wellness. The outpatient setting allows for a manageable caseload that prioritizes quality over quantity, facilitating thorough assessments and interventions. Such an approach is essential for optimizing patient outcomes within the constraints of modern healthcare systems. This position offers either a $5,000 sign-on bonus, or $7,500 in relocation assistance, and a travel stipend of $12,000.

Requirements

  • APRN or PA-C degree from an accredited program
  • Board Certification from an accredited program of your relative discipline
  • At least two years experience practicing as an N.P. or P.A in the primary care setting. Will consider experience in other care settings with experience caring for senior patients.
  • Must have valid driver’s license and insurance
  • Must be comfortable conducting home visits with medically complex patients
  • Ability to successfully complete Intermountain Health’s credentialing process

Responsibilities

  • Conduct primary care patient visits in the home setting
  • Experience with treating patients with multiple co-morbidities by assessing existing conditions; diagnosing new conditions and formulating plans of care.
  • Write referrals, renew medications, order diagnostics and manage results.
  • Advanced directive and advanced life care planning activities are a priority in this position.
