The Advanced Practice Provider works to support the mission and vision of DAP Health by providing excellent patient-centered care and services within a dynamic team environment. This role will provide direct healthcare services to patients under the direction and responsibility of our team of Physicians.

Requirements

  • Visual acuity and manual dexterity to interface with the computer and perform medical procedures as indicated
  • Ability to communicate effectively with all levels of individuals both internally and externally
  • Maintain CMEs/CEUs required for licensure
  • Master's degree from an accredited physician assistant or nurse practitioner program required
  • Valid CA Physician Assistant or Nurse Practitioner certification required
  • Valid CA DEA license required
  • Degree in Nursing
  • Minimum of one (1) year of experience as a Nurse Practitioner
  • Licensed to practice as a CA Nurse Practitioner and currently registered as a Registered Nurse in the State of California
  • Certified by ANCC or AANP
  • Valid BLS certification obtained through the American Heart Association or American Red Cross upon hire
  • Current State of California Physician Assistant License
  • Current valid Federal Controlled Substance Registration Certificate
  • Minimum of one (1) year of experience as a Physician Assistant
  • Valid BLS certification obtained through the American Heart Association or American Red Cross upon hire
  • Requires current and valid driver's license and current personal auto insurance
  • Ability to lift up to 40 pounds and move from place to place
  • Ensures compliance with policies and procedures related to safe work practices
  • Available for all shifts and, when required, able to work evenings and weekends

Responsibilities

  • Assess and interpret medical and psycho-social histories; perform physical examinations (including gynecological and breast examination as appropriate; identify problems; formulate a plan of treatment; provide education and referral (as indicated); and evaluate the effectiveness of the interventions prescribed
  • Independently assess and treat patients as clinically indicated. Consult with or refer clients with abnormal conditions or other medical, psychological, or social problems to the collaborating physician or another appropriate referral course when indicated
  • Perform or order laboratory tests and interpret the results as part of health management. Report communicable diseases in a timely manner to the appropriate Health Department as per health center protocol
  • Order medications
  • Follow established treatment guidelines as indicated by national guidelines or Health Center protocols
  • Meet clinical quality performance measures (UDS and HEDIS measures)
  • Conduct health teaching and counseling during the medical encounter, as appropriate, enabling the client to make informed choices about health and treatment options
  • Complete the client's clinical record documentation in a timely manner specific to the applicable policies and procedures of the health center
  • Participate in quality improvement/assurance activities
  • Participate in continuing medical education activities to enhance professionalism and to ensure quality of care
  • Provide on-call coverage as scheduled
  • Work collaboratively with all members of the health center to build a supportive team
  • Apply infection control measures according to Health Center Policies and Procedures
  • Maintain professionalism and adhere to client confidentiality
  • Perform other duties as assigned

Benefits

  • We are pleased to offer a sign-on bonus of $10,000 for this position. Half of the amount will be paid with your first paycheck, and half of the amount will be paid after 6 months of employment.
