Will initiate, modify and discontinue medications within a Collaborative Practice Agreement as defined by the California State Board of Pharmacy. Focus on management of complex disease states to help patients reach optimal therapeutic goals. Simplify patient medication regimens, minimize the risk of harmful medication interactions and educate patients to enhance adherence and clinical outcomes. Responsible for policy development, Continuous process improvement, continuing education, clinical and medical staff development and Pharmacy APPE student and resident preceptorship. Performs patient care assessments, orders and interprets laboratory tests, prescribes, modifies and discontinues medication therapies. Monitors clinical outcomes, communicates with medical providers, performs electronic documentation and refers patients to other health care providers. The Advanced Practice Pharmacist (APP) working in the Pharmacotherapy Clinic, is a clinical expert who provides comprehensive medication management for oncology patients. This role involves direct patient care, including assessment, education, and monitoring of complex chemotherapy and supportive care regimens. The APP collaborates with oncologists, nurses, and other healthcare professionals to optimize pharmacotherapy, improve outcomes, and enhance patient safety. Responsibilities include developing individualized treatment plans, managing adverse effects, ensuring adherence, and supporting transitions of care. The APP also serves as a resource for medication-related questions, participates in quality improvement initiatives, and contributes to the advancement of oncology pharmacy practice within the clinic.
