About The Position

The 12-month position prepares the AP Fellow to perform the duties of a primary care provider in a Federally Qualified Health Center with an emphasis on health promotion and disease prevention through patient education and counseling. Fellows are prepared to perform in an expanded role in the delivery of primary health care by receiving mentoring and a didactic program to increase skills.

Requirements

  • Education: Graduation within the last 24 months from an accredited NP or PA program. Must be able to obtain a State of California FNP or PA license, DEA and NPI number prior to onboarding.
  • Experience: As stated in Education.
  • Knowledge and Skills: Demonstrates basic knowledge of tasks on the most current AP Fellow competencies
  • Able to review medical history, assess mental health, complete physical exam.
  • Comfortable with use of Electronic Medical Record.
  • Demonstrates knowledge of compliance issues within the community clinic environment.
  • Demonstrates a willingness to report any incident that is unusual or incompatible with accepted clinic procedures.
  • Maintains confidentiality of patients at all times by complying with HIPAA policies.
  • Strong interpersonal skills; ability to be sensitive with persons of various social, cultural, economic and educational backgrounds.
  • Proficiency with Microsoft Office applications including Outlook, Word, Excel and PowerPoint.
  • Strong organizational skills with ability to prioritize projects, work relatively independently, manage multiple tasks, and meet deadlines.
  • Strong written and verbal communication skills.
  • Ability to work independently and as part of a team.
  • Good judgment, problem solving and decision-making skills.

Nice To Haves

  • Basic Spanish skills preferred (speak, read and write)

Responsibilities

  • Provide direct clinical care to patients at the health center in a full family practice scope.
  • Provide telehealth clinical care for primary care as directed by the health center and county health guidelines.
  • Work with mentoring staff to increase skills in clinical care, use of tools for decision support.
  • Attend didactic sessions to increase knowledge of clinical areas, FQHC organizational skills, leadership, EMR training.
  • Collaborate effectively with clinical team members in preventive care, treatment of acute and chronic illness.
  • Seek consultation as described in FNP/PA protocol.
  • Participate in ongoing training.
  • Other duties as assigned.
