About The Position

As an Advanced Clinical Pharmacist Medication Reconciliation, you will be responsible for the safe, effective, evidence-based, and cost-effective selection and utilization of medications for all patients. You will serve as a patient advocate when planning and implementing medication therapies and collaborating within the multidisciplinary team.

Requirements

  • B.S. or Pharm D. degree in Pharmacy is required.
  • Maintains current pharmacist licensure is required.
  • National Provider Identifier (NPI) is required.

Nice To Haves

  • Graduate of an accredited PGY-1 pharmacy residency program is preferred.
  • Graduate of an accredited PGY-2 pharmacy residency program is preferred.
  • 3+ years hospital pharmacy supervisory experience is preferred.

Responsibilities

  • You will work directly with the certified pharmacy technicians provide a streamlined medication reconciliation transition into the hospital.
  • You will ensure the medication history and reconciliation is completed according to established policies, procedures and protocols.
  • You will provide ongoing mentoring and training to ensure the competency of medication history and reconciliation staff.
  • You will ensure safe, appropriate, cost-effective medication therapies for patients, according to the established policies, procedures, and protocols.
  • As needed, you will provide recommendations and/or literature to prescribers supporting the use of formulary agent, as well as, education to clinicians and patients regarding medication management.
