Short Description Accurately validates prior-to-admission medication lists and takes medication & allergy history from patients. Contacts outpatient entities as needed for additional patient medication information. Communicates information to the health care team. Documents/corrects information in patient medication list in EPIC. Accurately and timely prepares all medications, including IV medications, under the direct supervision of the pharmacist as per policy and regulation. Completes expired medication inspections Answers phones to assist in dispensing questions Functionally uses the hospital information systems
