About The Position

The incumbent serves as an Advanced Medical Support Assistant assigned to a Specialty Care Setting involved in a specialty outpatient clinic model. The Specialty Outpatient Clinic care team consists of a Specialty care provider (MD, NP, PA), registered nurse care manager, clinical associate (LPN or medical assistant/health technician) and an administrative associate (MA/MSA/health technician).

Responsibilities

  • Plays a crucial role in coordinating with the specialty care outpatient team to assess clinic utilization, closely monitor clinic setup, and make necessary adjustments to effectively support clinic requirements.
  • Establish and maintain efficient communication channels with patients, interdisciplinary teams, VA medical centers, and external agencies to facilitate seamless transitions from inpatient to outpatient care, liaise with non-VA medical facilities, and manages and schedules Consult and Return to Clinic orders for patient follow up care needs.
  • Schedules patient appointments, provides quality customer service to meet the patient's needs, updates patient demographics and insurance information at time of check in,
  • Participates in team huddles and meetings to strategize and manage patient care, prioritize tasks, adjust workflow to meet clinic and patient care demands,
  • Oversees pre-appointment requirements for patient visits or procedures,
  • Engages in problem-solving discussions during team meetings,
  • Conducts administrative follow-up tasks,
  • Ensures continuity of care by collaborating with the medical team.
  • Processes patient secure messages through MyHealthy Vet,
  • Coordinates with the care team as necessary, and accurately provides documentation relevant patient care information into the system for efficient care coordination and delivery.
