About The Position

The Clinical Pharmacist oversees the medication distribution system for their area of responsibility. In addition, the Clinical Pharmacist identifies, prevents, and resolves drug therapy problems by ensuring each medication is indicated, effective, safe, and convenient for the patient.

Requirements

  • Doctor of Pharmacy
  • Completion of PGY2 Pharmacy Specialty Residency; OR Completion of PGY1 Pharmacy Residency PLUS 2 years of relevant experience; OR Completion of no residency PLUS 5 years of relevant experience
  • Pharmacist License in the State of Pennsylvania

Responsibilities

  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
  • Assures the appropriateness of drug therapy and compliance with hospital policies
  • Oversees the distribution of medications throughout TJUH
  • Supports implementation of processes to assure the quality of Pharmacy services
  • Trains pharmacy technicians, residents, students, interns and new pharmacists as assigned.
  • Precepts student pharmacists AND residents as assigned.
  • Knowledge of and ability to apply pharmacokinetic principles for monitoring and dosing medications
  • Monitor patient response to medications and provide recommendations on management if appropriate
  • Identify and document sentinel/serious events, near misses, medication errors and adverse drug reactions in the electronic event reporting system
  • Provide quality patient care in accordance with standards of practice, demonstrating high level expertise, competency, knowledge and skills
  • Promote and assure compliance with the hospital formulary and policies and procedures defined by the P & T Committee.
  • Provide evidence-based recommendations to support patient care
