About The Position
Advanced Practice Provider - Primary Care At Home (Full-Time) Salary: $108,000 - $135,000 annually based on years of experience Join Us and Be Part of the Samaritan Difference! As the Nurse Practitioner, you will be responsible for providing consultation, follow-up, and face to face visits to patients in multiple environments. This opportunity offers $108k-$135k annually based on experience and potential bonus incentives based upon Productivity and Performance goals. Requirements
- Experience in palliative care, oncology, hospice or geriatric care
- Master’s degree in nursing
- RN, APN license in NJ
- Proficiency on use of EMR systems

Responsibilities
- Perform follow up visit to patients in home settings and facilities
- Provide a health assessment appropriate to the visit being performed, as well as diagnose and treat patients who require management of symptoms
- Provide referrals/orders for lab work, testing, DME, homecare, PT, OT, etc. as appropriate
- Refer patients to other Samaritan services as appropriate
- Provide appropriate communication with patient’s specialists
- Complete documentation in EMR in timely manner and according to Samaritan policies
- Hospital and ED Discharges seen within 5 business days of discharge
- Represent Samaritan in a positive manner to patients, families and the community
- Support and participates in performance improvement and quality assurance programs including Value based/ACO program Measures
- Attend staff meetings and training courses/classes as required.
- Provide on-call coverage on rotating basis
- Other responsibilities as assigned
