As the Nurse Practitioner, you will be responsible for providing consultation, follow-up, and face to face visits to patients in multiple environments. Requirements

Experience in palliative care, oncology, hospice or geriatric care
Master's degree in nursing
RN, APN license in NJ
Proficiency on use of EMR systems

Nice To Haves

Bilingual is a plus!

Responsibilities

Perform follow up visit to patients in home settings and facilities
Provide a health assessment appropriate to the visit being performed, as well as diagnose and treat patients who require management of symptoms
Provide referrals/orders for lab work, testing, DME, homecare, PT, OT, etc. as appropriate
Refer patients to other Samaritan services as appropriate
Provide appropriate communication with patient's specialists
Complete documentation in EMR in timely manner and according to Samaritan policies
Represent Samaritan in a positive manner to patients, families and the community
Support and participates in performance improvement and quality assurance programs.
Attend staff meetings and training courses/classes as required.
Provide on-call coverage on rotating basis
Other responsibilities as assigned
