About The Position
Provide case management to 25 individuals on the case load at all times. Clients will have SMI/SUD diagnosis and possible chronic health conditions. Link and coordinate services for each client with community partners. Complete paperwork in electronic medical record.
Requirements
- Bachelors degree in approved Human Services field with 1 year post degree experience working in relevant field.
- Experience working with SMI/SUD population.
- Must have good communication and computer skills.
- Must be able to work independently and have excellent time management skills.
