Adult Targeted Case Manager (KRCC – Southern Counties)
About The Position
Title: Adult Targeted Case Manager Location: Kentucky River Community Care – Perry, Knott, Leslie, Letcher Counties Employment Type: Full-Time Primary Functions: Must be able to work independently and have excellent time management skills. Provide case management to 25 individuals on the case load at all times. Clients will have SMI/SUD diagnosis and possible chronic health conditions. Link and coordinate services for each client with community partners. Complete paperwork in electronic medical record. Requirements
- Bachelors degree in approved Human Services field with 1 year post degree experience working in relevant field.
- Experience working with SMI/SUD population.
- Must have good communication and computer skills.
