An Adult Targeted Case Manager is responsible for assessing, linking, planning, monitoring and advocating for recipients who have serious mental illnesses to provide for their care while living in the community. Case managers facilitate continuity of treatment for Peace River Centers catchment area recipient’s while at the in psychiatric, substance abuse and medical inpatient treatment and/or a state treatment facility and assist the recipients in accessing the service delivery system subsequent to their discharge. The target population served are persons who have been discharged from an inpatient setting, persons who are in need of linking and monitoring to prevent inpatient treatment, persons who lack the resources or ability to provide for their needs without assistance, and persons who have been arrested within the recent past.
