About The Position

The ASA Case Manager is responsible for providing person-centered care coordination for individuals with developmental and intellectual disabilities to promote a high quality of life. Ariel Clinical Services has been in business for over 30 years and has grown significantly since its early days of serving individuals with intellectual and developmental disabilities (IDD). Requirements

  • Bachelor’s degree in social or behavioral sciences, OR a combination of four years’ experience, and formal education in a Human Services field.
  • Ability to communicate respectfully and effectively in both individual and group settings.
  • Ability to be flexible and change direction rapidly with a positive attitude.
  • Ability to work independently, prioritize workload, and complete a wide variety of tasks in a timely and accurate fashion.
  • Basic computer knowledge.
  • Exercise independent, ethical judgment within the scope and boundaries of the department.
  • Ability to organize and present case material in a straightforward and concise written manner.
  • Ability to provide reliable, insured transportation for self and others.
  • Ability to demonstrate consistent and reliable attendance as scheduled and requested.

Nice To Haves

  • Experience with evidence based individual behavioral supports and services.
  • Minimum of two years working with I/DD population.
  • One year experience in mental health.

Responsibilities

  • Establish, build, and maintain rapport with individuals, independent contractors, and other members of the multidisciplinary team.
  • Provide case management for individuals and act as the liaison between collaborating agencies/multi-disciplinary team.
  • Develop, monitor, and document mandated services and programming, ensuring individual satisfaction.
  • Understand and participate in all relevant assessments to ensure appropriate level of care for individuals.
  • Facilitate home visits and meetings to assess the needs and progress of individuals and intervene as appropriate.
  • Effectively communicate potential risks and/or threats to an individual or program to supervisors and collaborating agencies.
  • Participate in mandatory rotation for 'on call' to support individuals.
  • Participate in ongoing program evaluation to ensure program effectiveness.
