About The Position

With general supervision, works from a trauma informed care perspective to perform a wide range of outreach activities related to advocacy, linkage and development of services for persons in need of those services. Individual should strive to create a healing environment that respects the perspectives and experiences of the individuals, families, staff and communities we serve by practicing safe, respectful communication as well as respecting individuals’ boundaries and differences

Requirements

  • Typical qualifications are a high school diploma, or equivalent, with two (2) years of relevant experience or an associate’s degree in human services or related field.
  • Valid NYS Drivers License as driving is an essential function of this position.
  • Thorough knowledge of community services within service area; general knowledge and understanding of mental illnesses, psychiatric rehabilitation model, and related issues; skills for independent living, has a basic understanding of the importance of working from a trauma sensitive perspective.
  • Use of contemporary office equipment, particularly a computer with word processing, database and report generating software
  • Ability to communicate effectively with diverse individuals
  • Ability to listen, understand and appreciate the experiences of clients

Responsibilities

  • Identifies the needs, problems, strengths, and functioning levels of clients
  • Interview, support, counsel, and assist clients with securing necessary services, living independently, and accomplishing their individual goals
  • Develop and maintain professional helpful and supportive relationships with clients, clients’ families, and significant others
  • May personally assist with paperwork, transportation, recreation, etc.
  • Monitor client functioning and communicate with service providers to assure that appropriate services are available and being utilized
  • Develop and maintain all records and reports as per organization policies and procedures
  • Attend meetings both in the agency and the community
  • Facilitate admission and discharge planning with inpatient programs
  • Link to resources in the community that may be beneficial
  • Research available resources and services throughout the county, and surrounding, that can be accessible to the clients and knowledge of how to access them
  • Complete all documentation within the required timeframes and submit monthly reports
  • Provide advocacy for the clients when situations arise
  • Provide outreach calls and visits to clients on caseload; meetings with clients should be face-to-face when possible, and should take place at home, in the community, or at the client’s preferred location.
  • Assist with linkage to benefits
  • Can effectively utilize Microsoft Office applications, navigate the internet, utilize program software and electronic health records for documentation.
  • Outreach to the community about the program and provide in-services to programs, providers, etc.
  • Knowledge of Medicaid’s eligibility requirements and the application process
  • Ability to effectively communicate with providers/care teams to ensure the client’s needs are met.
  • Complete documentation timely and accurately.
