About The Position

The Adult Guardianship Coordinator provides support for adults in need of a guardian to assist with the various requirements they must meet, including but not limited to legal, financial, and medical decisions. The Adult Guardianship Coordinator assists those that are deemed unable to live independently and have been legally determined to be incapacitated. The Adult Guardianship Coordinator (AGS Coordinator) plays an important role as the court appointed representative for adults who are unable to make informed decisions or to advocate for themselves. The AGS Coordinator acts as an agent of Thrive Alliance, who is the legal guardian, and carries out the duties required of the representative/guardian. The AGS Coordinator meets regularly with the individual based on the State requirements, or as needed, to ensure the individual is receiving quality care from care providers. The AGS Coordinator must ensure that the individual's finances are in order, they have the resources needed, and are receiving everything they should based on their care plan. Key Outcomes include: Presreening potential individuals for guardianship services, including assuring there are no other appropriate persons able to serve as guardian, obtaining relevant information about individual, presenting case to the Guardianship Advisory group, and coordinating with legal counsel to obtain court approved guardianship, Providing continuous oversight of all assigned individuals by: attending quarterly, annual, and care plan meetings, developing individual service plans in compliance with State and Agency policies, coordinating services where available, ensuring individual has adequate living arrangements in a safe environment, completing monthly reviews to monitor medication changes, behavior management plans, and to review any incidents, and reviewing/monitoring financial transactions, Maintaining individual records within State and agency policies and rules Participating in Adult Guardianship Advisory Committee Coordinating with the Volunteer Coordinator to recruit volunteers and the Human Resources Director to recruit interns, overseeing training of both groups.

Requirements

  • Bachelor’s Degree from an accredited educational institution is preferred; High School Diploma is required
  • Two years of experience in social services is required.
  • Currently hold state and/or national certification at the Registered Guardian level or required to attain the certification within 2 years of employment
  • Must possess a valid driver’s license and adequate auto insurance and ability to travel
  • Must possess adequate auto insurance to meet State requirements.
  • Must be self-motivated and able to work with minimal supervision
  • High level of organization and time management skills and ability to prioritize, organize tasks and time, and to follow-up as necessary
  • Average computer skills required to maintain required documentation and report functions
  • Able to juggle multiple projects and people, and meet multiple requests and deadlines
  • Empathetic and caring

Nice To Haves

  • Experience in positions of advocating for incapacitated adults is preferred.
