The Adolescent Treatment Services Coordinator position is part of the Child and Family Care Coordination Unit and employees in this role act as "Care Coordinators". Care Coordinators support children and families with care coordination, and link families to resources and services that are clinically necessary in a least restrictive environment to promote positive emotional and behavioral adjustment. Youth involved with Care Coordination have been identified as needing intensive mental health/behavioral health treatment and are referred to the unit for "Higher Level of Care" mental health services. These services are provided through service providers across a continuum of care ranging from community-based treatment to residential treatment. This position works closely with the family’s formal supports (e.g., child welfare, juvenile justice, schools, etc.) and informal supports (e.g., neighbors, family members, etc.) to improve the academic, social, emotional, and behavioral outcomes for their child(ren).
