About The Position

The Admissions Supervisor is responsible for the coordination, direction, organization, and supervision of the Admissions Team, including RNs, LPNs, Social Workers, and Pre-Admission Support Specialists. This role ensures timely patient admissions, resolves barriers to care, supports staff development, and fosters positive relationships with referral sources and community partners. The Admissions Supervisor ensures compliance with hospice regulations and maintains operational efficiency. Employees in this role is focused on home admissions. But must be willing and able to flex their work location between various hospitals and assist with home admissions as needed. Employees assigned this job title work as salaried employees understanding that length of workday is determined by workload. Employee must be able to flex hours as needed to meet referral and admission requests.

Requirements

  • Knowledge of hospice philosophy, services, admission procedures, and state/federal regulations.
  • Understanding of Advance Directives and the Medicaid application process.
  • Familiarity with pain and symptom management and interdisciplinary care.
  • Customer service, leadership, and supervisory skills.
  • Strong written and verbal communication.
  • Ability to guide families, assess values and preferences, and navigate complex decisions.
  • Skilled in electronic medical record documentation and Windows-based applications.
  • Strong clinical judgment, problem-solving, and prioritization abilities.
  • Minimum of two (2) years of experience in hospital or long-term care setting required.
  • Current Florida Registered Nurse (RN) license.
  • Current CPR certification.
  • Valid Florida driver’s license with acceptable driving record and reliable transportation.
  • Level II AHCA background screening.
  • Graduate from a diploma or associate degree nursing program; bachelor’s degree preferred.
  • Must be able to work flexible hours based on workload and admission demand.
  • Must be able to travel to hospitals and patient homes as needed.
  • Medical clearance and current immunizations required (TB, Hepatitis, Flu).

Nice To Haves

  • ICU or ER experience preferred.
  • CHPN certification preferred.
  • Graduate from a diploma or associate degree nursing program; bachelor’s degree preferred.

Responsibilities

  • Inform Director of Admissions regarding active referral status, risk management issues, treatment modalities and admission advancement status per hospital.
  • Coordinate daily staffing assignments based on referral volume, case complexity, and staff expertise to advance the admission process.
  • Collaborate with the Care Navigation Center to ensure timely and accurate referral intake.
  • Use data to improve referral quality, speed to care, conversion rates, and patient satisfaction.
  • Serve as a preceptor and mentor to Admissions Team members.
  • Provide same day service recovery and professional representation to referral sources.
  • Support quality improvement and compliance initiatives.
  • Monitor documentation compliance and adherence to internal standards.
  • Demonstrate critical thinking and independent problem-solving.
  • Maintain professional relationships with physicians, discharge planners, and healthcare providers.
  • Educate patients, families, and referral partners on hospice philosophy, services, and benefits.
  • Assess patients for hospice eligibility and conduct physical and psychosocial assessments.
  • Attends a minimum of 85% of the general staff and team meetings.
  • Promotes agency image within the community.
  • Develop initial plans of care and secure physician orders for medications, DME, and treatments.
  • Ensure comfort through timely coordination of care and appropriate nursing interventions.
  • Facilitate transfers and changes in level of care under Medicare Conditions of Participation.
  • Act as nurse designee while patient is hospitalized, ensuring updates to the care plan.
  • Provide training and feedback to staff on policies, procedures, and clinical performance.
  • Deliver education on Health Care Surrogate, Proxy, and Advance Directives.
  • Assist in professional development of staff and onboarding of new hires.
  • Participate in in-service programs, hospital committees, and outreach efforts.
  • Promote Community Hospice as a preferred provider and resource for early referrals.
  • Represent the organization professionally and ensure HIPAA compliance.
  • Comply with organizational policies and use resources responsibly.
  • Engage in continuing education and support professional development.
  • Perform other duties as assigned.
