About The Position

BAYADA Home Health Care is seeking a Full Time Admissions Registered Nurse to complete OASIS Start of Care (SOC) visits for adult and geriatric patients throughout northeast Massachusetts. This role is an integral part of a multi-disciplinary health care team, providing skilled nursing and rehabilitative care to clients, enabling them to remain at home. BAYADA emphasizes delivering the highest quality home health care with compassion, excellence, and reliability. The position requires strong assessment skills, experience with acute and post-acute care, and the ability to manage urgent clinical situations independently in a home setting. The nurse will also perform medication reconciliation, teaching, safety monitoring, and comprehensive wound assessment and care.

Requirements

  • Medicare Certified Home Health and OASIS documentation experience is required
  • Ability to work independently while knowing when to escalate concerns
  • Comfortable making real-time clinical decisions without immediate on-site support
  • Strong problem-solving skills in unpredictable home environments
  • Efficient with point-of-care documentation and technology
  • Strong attention to detail while maintaining productivity expectations
  • Flexible and adaptable to changing patient needs and schedules
  • Demonstrates cultural sensitivity and respect for patient autonomy
  • A current nursing license in good standing in the state of Massachusetts
  • Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
  • At least one year of current or recent hands-on medical experience within the past two years with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med surg, sub-acute care or rehab units (medical or skilled nursing settings)

Responsibilities

  • Complete OASIS Start of Care (SOC) visits for adult and geriatric patients
  • Use strong head-to-toe assessment skills with the ability to identify subtle changes in patient condition
  • Utilize experience with acute and post-acute care (e.g., CHF, COPD, wound care, IV therapy, post-surgical care)
  • Triage and manage urgent clinical situations independently in the home setting
  • Perform medication reconciliation, teaching, and safety monitoring
  • Perform comprehensive wound assessment, care, and documentation
  • Manage a dynamic visit schedule and prioritize urgent visits
  • Provide care in varied home environments
