About The Position

A Skilled Nursing Facility in Melbourne, FL is seeking an experienced Admissions Coordinator to manage hospital referrals, clinical reviews, and admissions conversion. This is a full-time, permanent role central to maintaining census stability and ensuring compliant, efficient admissions. The Admissions Coordinator works directly with the Administrator and clinical leadership to evaluate referrals, manage payer verification, and convert appropriate cases.

Requirements

• Minimum 2+ years of experience in SNF admissions or post-acute intake required
• Strong understanding of Medicare, Medicare Advantage, managed care, and Medicaid processes
• Experience reviewing hospital clinical documentation
• Proven ability to manage high referral volume and meet census goals
• Strong communication and relationship-building skills
• Experience with SNF EMR or admissions software

Nice To Haves

• Clinical background (RN, LPN, or Social Services) is a plus but not required.

Responsibilities

• Manage and respond to hospital and referral source inquiries
• Review clinical documentation and coordinate admission decisions with DON and leadership
• Verify insurance coverage, including Medicare, managed care, and Medicaid
• Communicate with discharge planners, case managers, and families
• Track referral sources and maintain accurate conversion reporting
• Ensure compliance with facility and payer admission requirements
• Support census growth and payer mix optimization
