About The Position

Ayers Health and Rehabilitation Center is seeking a compassionate, organized, and customer-focused Admissions Assistant to join our Social Services team. This role serves as a key liaison between residents, families, and the facility , supporting both the admissions process and ongoing social services needs. The Admissions Assistant supports the admissions and social services functions by assisting residents and families throughout the admission process, addressing psychosocial needs, and helping ensure continuity of care. This role requires strong communication skills, empathy, attention to detail, and the ability to manage emotionally sensitive situations with professionalism.

Requirements

  • High School Diploma or equivalent (required); healthcare or clerical experience preferred
  • Knowledge of: Geriatric patient needs
  • Medical and healthcare settings
  • Medicare, Medicaid, and private insurance processes
  • Ability to handle emotionally stressful situations, including illness, death, and family crises
  • Strong organizational, communication, and problem-solving skills
  • Self-motivated, positive, and team-oriented mindset
  • Proficiency with computer systems, office software, and standard office equipment

Responsibilities

  • Assist with the admissions process , including interviewing residents, families, and responsible parties
  • Obtain required admission documentation, signatures, and authorizations
  • Maintain inquiry and admission packets and ensure readiness for new admissions
  • Track and maintain accurate bed availability and census information
  • Take and manage new admission inquiries
  • Provide tours to prospective residents, families, and referral partners
  • Build and maintain positive relationships with residents, families, staff, and community agencies
  • Assist residents and families with advance directives and advance care planning
  • Ensure appropriate designation of healthcare decision-makers for incapacitated residents
  • Provide basic casework and counseling support, encouraging family involvement
  • Refer residents and families to appropriate community or social service agencies as needed
  • Complete required documentation, including admission notes, social histories, and progress documentation
  • Document and communicate significant concerns or events to leadership
  • Assist with discharge planning, including transfer/discharge letters and Non-Coverage notices in accordance with federal regulations
  • Maintain an up-to-date community resource guide
  • Coordinate with the business office and medical records to support accurate census and managed care billing
  • Participate in interdisciplinary meetings and facility committees as required
  • Maintain close communication with the Admissions Director, Social Services Director, and Administrator
  • Participate in continuous quality improvement efforts
  • Support a positive public relations presence for the facility within the community
