About The Position

The Transitions of Care Nursing Supervisor is charged with overseeing both clinical and administrative aspects of nursing services during assigned shifts. This leadership role ensures the delivery of high-quality resident care through effective clinical oversight, staff development, and coordination of care transitions. The Transitions of Care Nursing Supervisor is tasked with directing nursing operations, promoting clinical excellence, and supporting the professional growth of nursing staff. The TOC Nursing Supervisor plays a critical role in managing seamless transitions for residents during admissions, discharges, and transfers, ensuring continuity and safety of care. This role also involves conducting comprehensive medication reviews and providing clear, individualized instructions to residents and their representatives, ensuring all care needs are fully understood and supported.

Requirements

  • Effective and efficient communicator- Proficient in oral and written communication across diverse audiences.
  • Proficiency in technology and systems management – Office Suite (Microsoft), POC, databases
  • Customer Focused Professional- known for delivering warm, attentive, and solution-oriented service to clients and team members.
  • Highly Organized and Detail Oriented – proficient in accurate, and timely documentation including medication reconciliation and transition notes.
  • RN License in Pennsylvania
  • Minimum of 3 years nursing experience with a minimum of (1) year experience in gerontology and (1) year supervisory experience preferred but not required

  • Bachelor’s Degree Preferred, not required

Responsibilities

  • Assist the clinical managers in the daily facilitation and leadership of clinical rounds
  • Assists in ensuring residents receive care and services that meet their needs through ongoing assessment and evaluation of interventions based on accepted standards of practice
  • Facilitate communication and task follow-up with the interdisciplinary team to ensure continuity of care and alignment on resident needs
  • Conduct routine rounding to address resident and staff needs, assist with direct care tasks as needed, and act as a resource for clinical and operational decisions during the shift
  • Oversee nursing staff performance, provide guidance, and initiate disciplinary actions as appropriate to ensure high standards of care and professionalism
  • Participate in interviewing, hiring recommendations, and conducting new hire check-ins to support effective recruitment and retention
  • Assess staff performance, complete evaluations, and conduct disciplinary actions or counseling as needed in alignment with organizational policies
  • Encourage personal and professional growth through mentorship, education, and performance feedback
  • Provides as-needed assistance with the orientation and onboarding of new hires and agency staff to ensure smooth integration into the team
  • Completes fit testing training and is qualified to conduct fit testing for team members in compliance with safety standards
  • Effectively manage multiple projects to completion on time and on budget
  • Develops and monitors budget based on professional experience and data
