About The Position
Admissions Nurse – Licensed Vocational Nurse (LVN) Per Diem Willow Creek Healthcare Center – Clovis, CA Skilled Nursing Facility (SNF) Competitive Pay Willow Creek Healthcare Center is seeking a detail-oriented and compassionate Licensed Vocational Nurse (LVN) to serve as our Weekend Admissions Nurse. In this important role, you’ll be the first clinical point of contact for new residents and their families, helping ensure a smooth and supportive transition into our care. Requirements
- Current California LVN license in good standing
- CPR certification (required)
- Knowledge of state and federal SNF admission regulations
- Excellent communication, organization, and documentation skills
- Professional, compassionate, and resident-centered approach
Nice To Haves
- Previous admissions or skilled nursing experience preferred
- Familiarity with EMR systems (PointClickCare a plus)
Responsibilities
- Coordinate and manage all weekend admissions into the facility
- Complete initial nursing assessments, documentation, and vital signs upon resident arrival
- Communicate with hospitals, physicians, and family members to ensure proper continuity of care
- Collaborate with interdisciplinary teams to develop and implement initial care plans
- Ensure all admission documentation is accurate, timely, and meets regulatory requirements
- Provide a warm and welcoming introduction for new residents and their families
- Support and guide CNAs and LVNs involved in the admission process
- Assist with nursing duties on the floor as needed
