Requirements
- Licensed as a Registered Nurse in the state of Minnesota.
- Driving is required; must have a reliable vehicle with current insurance and a valid Driver's License.
- Prior Home Health Nursing experience.
- Proficiency with Medicare & OASIS documentation.
- Demonstrated ability to work autonomously.
- Excellent at planning, organizing, and communicating.
Responsibilities
- Perform Admission / "Start of Care" OASIS Visits throughout an area of the Metro.
- Evaluate and assess clients for home health services and create their plan of care.
- Recommend services from other disciplines that have not yet been ordered by the physician.
- Collaborate with clients, families, physicians, and other members of the home care team.
