The Hoy Center at Westminster-Canterbury on Chesapeake Bay takes a holistic approach to skilled nursing care and aging. Our person-centric living aligns the delivery of care with individual needs and preferences. Skilled Nursing/Memory Care/Rehabilitation To learn more: https://www.wcbay.com/services/skilled-nursing/ The Admissions Manager is the first point-of-contact for families, discharge planners and other new, short stay referral sources that are interested in placement in our Stone Rehabilitation Center. The Admissions Manager leads our results-driven census metrics and is responsible for meeting established census & business development goals. Requirements

  • 1-2 years experience in healthcare field and/or sales and marketing
  • Valid drivers license and proof of insurance.
  • Experience with Microsoft Office and Google Suite.
  • Flu-Vaccinated

Nice To Haves

  • Bachelor's Degree in healthcare-related field.
  • 1-2 years of supervisory experience.
  • Prior clinical experience in a Skilled Nursing or Long Term Care environment.
  • Previous experience supporting or participating in business development initiatives.
  • COVID - 19 Vaccinated . As of now, the 2025–2026 COVID-19 vaccine formulation is being distributed to providers and pharmacies; however, it has not yet received official approval from the CDC. In Virginia, pharmacists may administer COVID-19 vaccines under the Virginia Board of Pharmacy’s statewide protocols or with a direct order from a licensed medical provider. We encourage all new hires to consult with their healthcare provider regarding the most current vaccination recommendations. Please be aware that, depending on future CDC guidance or regulatory updates, we may require proof of the most recent COVID-19 vaccination as part of our ongoing commitment to health and safety.

Responsibilities

  • Review and manage all referrals for admissions by screening patients and comparing patient's condition to admission criteria.
  • Evaluate and accept or reject patients; referring patients and families to other programs and institutions.
  • Maintain frequent and open communication with local hospitals and referral sources regarding patient referrals.
  • Request any further information necessary to ensure a smooth transition of care from referring agency to The Hoy Center/Stone Rehab Center.
  • Manage internal communication and admission processing that promotes excellent customer service.
  • Obtain patient information from referral sources, patient, and/or family as needed; verify information for accuracy.
  • Review/complete admission contracts with new admits.
  • Complete Electronic Health Record (EHR) tasks in a timely manner.
  • Create new strategies to increase the growth of the census and drive customer satisfaction.
  • Promote and enhance The Hoy Center/Stone Rehab programs through marketing and community outreach initiatives that support organizational business objectives.
  • Maintain and update required Admission reporting as requested by Healthcare Administrator.
  • Support compliance with regard to regulatory requirements and bundle partnerships.
  • Participate in on-call rotation to support admissions 7 days a week to support business goals.
  • Perform other duties as assigned.
